Nurse Practitioner Care, Scope of Practice, and End-of-Life Outcomes for Nursing Home Residents with Dementia

JAMA Health Forum – JAMA Network; by Cyrus M. Kosar, PhD; Bishnu B. Thapa, MPA, PhD; Ulrike Muench, RN, PhD; Christopher Santostefano, RN, MPH; Emily A. Gadbois, PhD; Hyesung Oh, MA, MBA; Pedro L. Gozalo, PhD; Momotazur Rahman, PhD; Elizabeth M. White, APRN, PhD; 5/10/24

Question: Is nurse practitioner (NP) care associated with end-of-life outcomes for nursing home residents with Alzheimer disease and related dementias (ADRD), and do these associations differ between states with full vs restrictive NP scope of practice regulations?

Findings: The results of this cohort study including 334 618 US nursing home residents with ADRD indicated t hat decedents with greater NP involvement at end of life had fewer hospitalizations and higher hospice use. The adjusted differences in outcomes between decedents with extensive vs minimal NP care were larger in states with full scope of practice regulations than in states with restrictive regulations.

Palliative care Advanced Practice Registered Nurses (APRN)s are specialists and leaders in disease-modifying care, pain and symptom management, the highest quality of life possible, and compassionate end-of-life care. Although HPNA has a wonderful reputation for offering resources and support for hospice and palliative nurses, most of these resources were not geared specifically for APRNs.

The Hospice and Palliative Nurses Foundation (HPNF) received a generous grant from the Y.C. Ho/Helen & Michael Chiang Foundation to provide support and resources for hospice and palliative care APRNs. This grant was designed to provide access to networking, resources, and educational opportunities that will empower APRNs and result in better care and improved quality of life for patients and families dealing with serious illness. These resources may be especially valuable to APRNs practicing in a setting with limited resources.

Team Conference for Hospice & Palliative Care

The Power of 1 FTE Session
at the 2024 Team Conference for Hospice & Palliative Care

Presented by: Caitlin (Cate) Carl, BSN, RN, Anna Engle, MSN, RN, and Mark Houghton, MSN, AGACNP-BC

A Critical Access Hospital (CAH) is a rural hospital certified by Medicare to provide essential access to high-quality health care in communities without access to larger facilities. It can be challenging to provide palliative care in a CAH due to lack of resources.

This session will discuss how to start and maintain a palliative care program in a CAH.  The speakers will discuss how to build a sustainable program that meets the unique challenges of rural communities with limited resources from development to implementation to continuing community education.
