HPNA and the Social Work Hospice and Palliative Care Network (SWHPN) are pleased to welcome Transforming Chaplaincy as a host for the 2023 Team Conference for Hospice and Palliative Care #TC4HPC.

Transforming Chaplaincy is a product of a pioneering partnership with Rush University Medical Center, Brandeis University, and the Joint Research Council. Transforming Chaplaincy serves a wide variety of diverse communities – chaplains, educators, and health care administrators and professionals – by enhancing their practice with the best research and scholarships available. Their mission is to promote research literacy in chaplaincy to improve patient outcomes.

The Team Conference brings the interdisciplinary hospice and palliative care team together to explore best practices that enhance the specialty and improve patient and caregiver outcomes. This conference aims for colleagues to connect with one another and leave feeling rejuvenated and inspired in their own journey as a health care provider.

The 2nd Annual Team Conference will take place virtually on September 28-29, 2023. NCPD and CME credits will be offered.
