Eunita has over 25 years of experience in home health and hospice and provides education and training on end-of-life care. Eunita currently serves as hospice administrator for Encompass Health - Hospice and holds several specialty certifications including CHPN, Certified Hospice Administrator, and ELNEC Trainer. Eunita is a member of the Alabama State Nurses Association, Alabama Hospice and Palliative Care Organization, and HPNA where she serves as Southeast District Chapter Leader, chair of the RN Professional Development Advisory Council, and past-President of HPNA's Central Alabama chapter. 

About the Webinar
It is well appreciated that patients have better outcomes and higher satisfaction when care is effectively coordinated. The transition of care from an inpatient setting to the community often lacks the coordination needed to ensure the delivery of high-quality care that meets the needs of patents and families. This often leads to re-hospitalizations, unnecessary care, and extensive costs. The purpose of this webinar is to facilitate improvements in transitions of care from palliative inpatient settings to the community and to facilitate the development of processes to effectively manage patient care following acute care discharge.
